How to File: Online

To file your form online, you must access Massachusetts’ online portal. You will need a Customer ID Number and a PIN. These would have been assigned to you when you formed your business. If you have forgotten them, you will need to contact the Secretary of the Commonwealth’s office.

We are not required to provide a signature for this form.

How to File: Fax

To fax your form, you must first create a Fax Voucher Coversheet, which can only be done on the Secretary of the Commonwealth’s website. You must select the filing and fill in the proper information. Then you must pay online (there is a $5 expedite fee for all Fax Filings). You can pay using Visa, MasterCard, or an E-check.

After payment, print your Coversheet and fax your COMPLETE filing (the proper form and Coversheet) to: 617-624-3891.

Faxed filings will be processed by the next business day.

How to File: Mail or Walk-In
